Abstract: 

Background: Concerning the importance and the high frequency of renal involvement in patients with SYSTEMIC LUPUS ERYTHEMATOSUS ((SLE)), and the ability to measure the value of hematuria on predicting the incidence of LUPUS nephritis, this study aimed to determine the frequency of LUPUS nephritis in (SLE) patients with hematuria. Methods: This cross-sectional study investigated 500 patients with (SLE) in Isfahan City, Iran, between the years 2001 and 2015. Medical history, as well as demographic and clinical data of patients including the levels of C3, C4, and anti-dsDNA antibody, presence of LUPUS nephritis, proteinuria, cellular cast, pyuria, urinary tract infection, urinary tract stones, congenital renal defects, and history of any kidney disease were obtained from the patients' files at the end of the study period, and analyzed using SPSS software. Findings: The frequency of hematuria in patients with LUPUS nephritis was 36. 1%, and there was no significant relationship between the presence of hematuria and the existence of LUPUS nephritis in patients with (SLE) [odds ratio (OR) = 0. 727, confidence interval of 95% = 0. 376-1. 648, P = 0. 520]. The individuals with LUPUS nephritis had a lower age distribution compared to the cases without it (P = 0. 005). Most of the cases in the LUPUS nephritis group had low C3 and C4, but most patients in non-LUPUS nephritis group had normal levels of C3 and C4 (P < 0. 05). Conclusion: The relative frequency of LUPUS nephritis in (SLE) patients with hematuria was approximately 36%. Therefore, the presence of hematuria in patients with (SLE) may account for incidence of LUPUS nephritis one-third of the cases.

Abstract: 

Background and aims: Nephritis is one of the most dangerous presentations of (SLE). LUPUS in pregnancy can increase maternal and fetal mortality. In this report a pregnant woman with LUPUS nephritis is presented.Case report: A thirty year old woman in 30th week of pregnancy was admitted to internal medicine ward for evaluation of peripheral edema and hypertension. Important findings of her laboratory exam were: azotemia, hematuria, proteinuria, anemia, large size kidneys, active urine sediment, pleural and pericardial effusion, ascites, high titer of ANA, anti dauble strand DNA and LUPUS anticoagulant. Renal biopsy was done, pulse of methyl prednisolon prescribed and hemodialysis was started. After delivery, pulse of cyclophosphamide was also given and her renal function improved.Conclusion: The patient had not history of rheumatologic disease and the first presentation of LUPUS was severe nephritis during pregnancy. Fortunately, the response of patient to the treatment was good. After about 3-years follow-up, patient’s renal function is normal and her son seems healthy.

Abstract: 

and the lack of comprehensively categorized information in Iran, this study was performed to investigate the prevalence of different oral manifestations and report their possible associated factors in patients with (SLE). Materials & Methods: This cross-sectional study was performed on 96 (SLE) patients referred to two rheumatology clinics in Yazd, Iran, from September 2020 to February 2021. (SLE) patients were diagnosed based on the last revision of American College of Rheumatology (ACR) criteria in 1997. A questionnaire was created to collect demographic information and oral health status. Data were analyzed using SPSS v20. 0 and p-values of P < 0. 05 were considered statistically significant. Results: This study included 13 men (13. 5%) and 83 women (86. 5%), with an average age of 31. 9 ±, 11. 35 years. Oral lesions were diagnosed in 64. 4% of patients, with white and red lesions being the most frequent (58%), and the most common region involved was buccal mucosa (25. 8%). 97. 1% of participants had caries and 86. 5% had periodontal diseases. There was a significant association between the presence of oral lesions and female gender, longer duration and higher activity level of the disease and simultaneous presence of periodontal disease and missing or filled teeth. (P < 0. 05). Conclusion: Given the frequency of oral lesions in more than 60% of patients, as well as the high incidence of caries and periodontal disorders, regular oral examinations in these patients appear to be particularly important.

Abstract: 

LUPUS ERYTHEMATOSUS (LE) is an autoimmune disease. The mortality rate and causes of death are different in various countries. The present study is done to find out the causes of mortality in the SYSTEMIC LUPUS ERYTHEMATOSUS patients in LUPUS unit of Rheumatology Research Center and other wards of Shariati Hospital. This study retrospective according to medical records of patients who were referred to LUPUS Unit of Rheumatology Research ICU, Gynecology and Nephrology wards of Shariati hospital during 10 years, from 1991 till 2001. Of 2021 recorded patients files, 165 (7.8%) deaths were already recorded. 84.8% were female and 15.2% male. The duration of disease was 36±12 months. The mean duration of follow up from entering the study till time of death was 25.9±6.8 months. The causes of mortality were as follows: infection (12.1%), respiratory (8.5%), CNS (13.9%), kidney (3.6%), malignancy (1.2%), unknown (7.8%), active LUPUS (3%). Infection accompanied with other causes (27.9%), respiratory with other c (11.5%), CNS with other causes (5.5%) and renal with other causes (4.8%). Infection is the most common cause of death in (SLE) Iranian patients, which is compatible with some multicentric studies. But in some previous studies myocardial infarction was reported as the main cause of death.

Abstract: 

Background: SYSTEMIC LUPUS ERYTHEMATOSUS ((SLE)) is a common autoimmune disease, and if not diagnosed and treated in a timely manner, can lead to irreversible complications such as LUPUS nephritis. Thus, the early detection of the disease is very important. On the other hand, some studies have shown that there is a relationship between the serum levels of anti-Mullerian hormone (AMH) and the activity of LUPUS disease. The aim of this study was to determine the relationship between antibody titer with severity and activity level of (SLE). Methods: In a cross-sectional study, 70 women in age range of 18-35 years old with (SLE) who referred to rheumatology clinic of Alzahra hospital, Isfahan, Iran, were selected. LUPUS activity was determined using (SLE) Disease Activity Index-2000 ((SLE)DAI-2k) score, and serum level of AMH was measured; and the relationship between (SLE) activity and serum level of AMH was evaluated. Findings: Mean serum level of AMH was 1. 49 ± 1. 58 ng/ml, ranging from 0. 20 to 7. 36; and thus, 11 patients (15. 7%) had low levels of the hormone. Moreover, there was a statistically significant inverse correlation between disease activity scores and serum levels of AMH, as much as 0. 46 (P < 0. 001). Conclusion: The results of our study showed a relation between LUPUS activity and serum AMH. Especially, among the patients under treatment with corticosteroids and cyclophosphamide pulse therapy, there were lower levels of AMH; and this might lead to ovarian reserve loss and fertility problems in women of childbearing age.

Abstract: 

SYSTEMIC LUPUS erythemathosus is an autoimmune disease that affected many various types of tissues in 10% of world population and over 30 genes has associated with it. Nouroimmunoendocrynology concepts have shown that immune system could be affected by neuron system and vice versa, 5-hydroxytryptamine receptor a (5HT3Ra) was studied as a main receptor in these relations. In this study, peripheral blood sample were collected from ((SLE)) patient and normal individuals. The total cellular RNAs were extracted and the cDNAs were synthesized. This process was followed by real-time PCR using specific primers for 5HT3Ra gene and beta-actin gene as internal control. Eventually PCR products have been sequenced. Results of this study suggested that this special receptor expressed in polymorpho-nuclear cells. We found over expression of 5HT3Ra in patients in comparison with healthy individuals group. Interestingly, some nucleotide changes have been found in 5HT3Ra gene in patients but not found sequential nucleotide changes in healthy individuals group. This study supposed that over expression of 5HT3Ra gene in (SLE) patients lead to over activation of immune cells that derived from over stimulation of them from serotonin blood serum that finally lead to autoimmune reactions that terminated in (SLE).
